About the Role
Working in the Heart Lung Stream, providing social work services to inpatients, families, and carers within Cardiology, Cardio-Thoracic Surgery, Thoracic Medicine, Thoracic and Cardiac Rehabilitation and Heart and Lung Transplant including psychosocial assessment, risk assessment, counselling, and family support.
Responsibilities
- Participate in multidisciplinary team meetings to support patient care, discharge planning, and coordination of referrals to community supports, accommodation, and placement services
- Advocate for patients and carers, including support with NCAT Guardianship and NDIS applications where required
- Contribute to clinical operations including documentation, handovers, statistical reporting, clinical cover, on-call duties, and resource development
- Engage in professional development activities including supervision, education, research, student training, quality improvement, and mandatory training compliance
Requirements
- Hold a degree in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW), with demonstrated clinical experience in a tertiary hospital or health-related setting
- Strong clinical skills and knowledge in psychosocial assessment, counselling, therapeutic interventions, case management, advocacy, and complex discharge planning
- Understanding of and practical experience in crisis intervention, short-term solution-focused approaches, and grief and loss support
- Ability to work effectively within multidisciplinary teams in high-pressure environments, managing competing demands with strong negotiation and liaison capabilities
- Willingness and ability to participate in the after-hours on-call hospital social work crisis service as required
