About the Role

The Pharmacist is responsible for delivering comprehensive pharmaceutical care through the medication use process, ensuring safe, accurate, and effective medication therapy for patients. This role oversees pharmacy technicians and interns, coordinates workflow, verifies and approves medications prior to dispensing, and maintains thorough documentation of patient care activities. The Pharmacist assesses patient-specific needs, interprets clinical information, and provides age-appropriate pharmaceutical care while maintaining compliance with all applicable local, state, and federal pharmacy regulations.

Responsibilities

  • Processes medication orders and evaluates their appropriateness and completeness with respect to patient data including age, height, weight, diagnosis, laboratory values, and other pertinent patient information.
  • Utilizes clinical pharmacy skills to assess patient medication regimens and formulates recommendations to ensure optimal safety, efficacy, and cost-effectiveness for all prescribed medication therapies.
  • Contacts the prescriber with suggestions for improvement and resolves problems with medication orders as needed.
  • Serves as a resource for patients and the health care team.
  • Provides requested medication information in a timely manner.
  • Complies with directives of the Pharmacy and Therapeutics Committee/Clinical Consensus Group.
  • Documents activities related to patient care in the electronic medical record and other electronic databases as applicable (e.g. pharmacist to pharmacist/provider communication, adverse drug event reporting, and clinical interventions).
  • Drives their professional development through the utilization of BPS educational resources to achieve competence in the current and future BH acute pharmacy core clinical practices and activities.
  • Assist with training and onboarding of new staff as applicable.
  • Prepares and dispenses medications, including sterile or non-sterile compounded medications, in an accurate and timely manner in compliance with company policies and procedures and established regulatory requirements.
  • Coordinates medication preparation and distribution and ensures accuracy prior to dispensing medications.
  • Oversees the day-to-day operations of the pharmacy, which may include management of technology, inventory management, and accurate record keeping.
  • Supervises and directs the work of pharmacy technicians, interns, and students.

Requirements

  • Requires unrestricted Pharmacist licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy.
  • Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.
  • If not licensed in the state of practice, must have completed the Board of Pharmacy application process within 30 days of job offer.
  • Must be licensed within 90 days of hire unless the delay is due to the State Board of Pharmacy.
  • Banner Registry and Travel positions require a current and unrestricted Pharmacist license (not intern) at the time of hire and a minimum of one year experience in an acute care hospital setting.
  • Experience must include working in an acute care setting within the past 12 months as a pharmacist in the specialty area.
  • Must possess good organizational, problem solving, and computer skills.
  • Must possess good written and verbal communication skills for interfacing with all levels of staff, physicians, patients and other contacts.
  • Must be able to use reference tools to evaluate patient care and answer questions posed by customers.
  • Must have knowledge of computer software, particularly medication management information system software, Microsoft applications and Automated dispensing system.
  • Employees working at Banner Behavioral Health Hospital must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
  • An Arizona Criminal History Affidavit must be signed upon hire.
