About the Role

The Behavioral Health Supervisor provides administrative and clinical supervision to Patient Navigators and other behavioral health staff within Ward 86. This role focuses on planning and supervising patient coordination services for individuals living with HIV, at risk of HIV acquisition, and those with high psychosocial complexity. The position involves collaborating with a multidisciplinary team, managing operations, and potentially providing direct patient services.

Responsibilities

  • Provide administrative and clinical supervision to Patient Navigators and other behavioral health staff.
  • Screen and assess patients for service needs, psychosocial history, and presentation.
  • Develop and execute care plans.
  • Oversee contract compliance and deliverables.
  • Supervise patient coordination services operations within assigned clinical programs.
  • Collaborate with clinical leads and a multidisciplinary management team.
  • Provide leadership and guidance on clinical matters.
  • Handle administrative responsibilities such as setting work schedules, resolving system issues, and managing complex cases.
  • Provide direct services to patients with complex psychosocial needs, including those experiencing homelessness, substance use, and mental illness.
  • Work collaboratively with internal care team members and external organizations.

Requirements
