About the Role

The Support at Home Program builds an integrated, technology-enabled care model prioritizing customer choice, safety, and continuity. Responsibilities

• Lead the delivery of strategic initiatives supporting the implementation and enhancement of the Support at Home program.
• Drive projects across workforce innovation, service capacity, and operational improvement.
• Embed new ways of working and deliver meaningful transformation across the business.
• Deliver projects and continuous improvement initiatives supporting the growth of the Support at Home program.
• Drive operational, workforce, and service delivery improvements.
• Manage project plans, risks, and stakeholder relationships.
• Partner across clinical, operational, and technology teams to implement change.
• Track outcomes and identify opportunities to improve customer experience and performance.

Requirements

• Demonstrated experience delivering projects within the healthcare, aged care, or community care sector.
• Strong stakeholder management and influencing skills, with the ability to work across all levels of the organisation.
• Experience driving business process improvement and implementing change.
• Able to work autonomously while building strong collaborative relationships across teams.
