About the Role

Atria Health Institute is seeking a Nurse Practitioner or Physician Assistant to partner with physicians in providing exceptional care within our state-of-the-art facility and to members in their homes. This role involves collaborating with CMOs on preventive care, well visits, and comprehensive health assessments, as well as providing supervision for urgent home care visits. The position requires flexibility to work weekends and off-hours for on-call coverage and adaptability to work across multiple disciplines including Internal Medicine, Cardiology, Neurology, and Geriatrics. Responsibilities

Partner with Atria physicians to provide exceptional care in a state-of-the-art facility and to members in their homes.
Collaborate with CMOs to provide Preventive Care, Well Visits & Comprehensive Health Assessments.
Provide direct and indirect supervision for urgent Home Care visits.
Work fluidly and competently across multiple disciplines, including Internal Medicine, Cardiology, Neurology and Geriatrics for cross coverage.
Adapt to gain training and demonstrate competencies in a variety of Point of Care Testing modalities and devices.
Perform clinical, administrative and clerical tasks outside routine scope of practice.
Provide Home Care Services for minor procedures, such as insertion of peripheral IV for medication administration, wound care, vaccinations, suturing and phlebotomy techniques.
Document and maintain Medical Records for members.
Conduct Telehealth visits and electronically prescribe medications.
Perform EMR data integration.
Participate in initiatives such as committees for Quality & Patient Safety, Protocol & Procedure Development and Review Processes.
Maintain a high-quality workforce to assist in achieving the Organization’s Mission.
Maintain Professional Licenses, Certifications, DEA certification, State requirements and National Certifying Agencies credentials.
Participate in appropriate continuing medical education programs.

Requirements

Current License required as NP for specialty, or current Licensed PA.
2+ years’ experience as practicing NP or PA.
ACLS, PALS and BLS certification from American Heart Association.
Capable and experienced with relevant procedures for proficiency in delivering care in urgent care and emergency medicine setting.
Available for variable weekend on-call assignment.
