About the Role
Crestwood is seeking a Service Coordinator (Mental Health Case Manager) to join their team. This role involves assessing clients, coordinating treatment plans, and facilitating discharge planning within a behavioral health setting. The organization emphasizes a personal and self-directed recovery process, empowering clients to develop skills for thriving.
Responsibilities
- Assess clients to identify individual problem areas necessitating treatment and complete assessments.
- Coordinate interdisciplinary team meetings and discharge planning efforts.
- Coordinate the overall treatment plan implementation for the client on the unit.
- Work closely with nursing staff to ensure continuity of treatment efforts.
Requirements
- Two (2) years of full-time experience or part-time equivalent in a mental health setting.
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
- Familiarity with regulatory standards.
- Employment is contingent upon successful completion of a background investigation including criminal history and identity check.
- Crestwood is required to participate in the E-Verify program.
