About the Role
Community Health and Wellness, LLC provides in-home and community services for individuals with disabilities and other related conditions. This role involves creating tailored goals, conducting initial evaluations, and implementing patient care plans.
Responsibilities
- Create tailored goals to meet the needs of individual clients.
- Turn in timely documentation including weekly invoicing, progress notes, quarterly reports, and related paperwork.
- Conduct Initial Evaluations (as needed for clients new to therapy), writing goals and objectives, and determining a recommended frequency of services.
- Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader.
- Implement patient care plans through goal-related activities.
Requirements
- Must be able to travel to current or prospective clients’ homes.
- Must be able to communicate with health care professionals, clients, families, and the general public through verbal and written communication.
- Must have access to a private personal laptop or desktop computer for HIPAA compliance.
- Able to work independently, manage one’s own time and the time of client(s).

- LMT Certification
- CPR/AED Certification (American Red Cross or American Heart Association only)
- License to practice within the state of Texas
- Driver's License
- Social Security Card
- Professional Liability Insurance
- All contractors must submit to background and reference checks.
