About the Role

Provides clinical behavioral health services to all age patients and families within Apicha Community Health Center, including assessments, diagnosis, counseling, psychotherapy, and crisis intervention.

Responsibilities

  • Conduct psychosocial assessments, assign diagnoses according to DSM 5 and ICD 10, create and implement treatment plans.
  • Facilitate related services for patients while respecting confidentiality and privacy.
  • Provide treatment services to an assigned caseload, managing utilization and billing targets.
  • Offer psychotherapy and psychoeducation using individual, family, and group models.
  • Involve patients in their treatment planning and education about their illnesses.
  • Provide crisis intervention, including escorting patients to the emergency room and developing safety plans for at-risk individuals.
  • Collaborate with primary care providers and make referrals as needed.
  • Participate in multidisciplinary team meetings and case conferences.
  • Complete charting in the electronic medical record (EMR) and concurrent documentation in a timely manner.
  • Contribute to Quality Improvement and Quality Assurance activities.
  • Participate in the development and review of behavioral health policies and procedures.
  • Practice within the scope of license, training, and credentialing, adhering to standards of care.
  • Produce required reports in a timely manner.
  • Perform other duties as required or assigned.

Requirements

  • Must hold a valid New York State license to provide mental health services (LCSW, LMHC, LMFT).
  • Possess an advanced degree in mental health counseling, social work, or a related field.
  • Experience working with target populations is strongly preferred, including individuals with serious mental illness, developmental disabilities, serious substance use disorders, individuals at high risk of or living with HIV/AIDS, chronic illness, Asians and Pacific Islanders, LGBTQ+, and immigrants.
  • Strong organizational skills to manage patient assessments and treatment plans.
  • Experience in maintaining accurate electronic medical records and documentation.
  • Excellent written and verbal communication skills.
  • Ability to build rapport quickly.
  • Ability to communicate with compassion and empathy.
  • Detail-oriented.
  • Ability to work effectively in both collaborative and individual settings.
  • Ability to work with people from diverse backgrounds in a multicultural setting.
  • Ability to work with Limited English Proficient populations and interpretation support.
  • Proficiency in Microsoft Office; ability to enter data accurately.
  • Ability to work flexible hours, including some evenings and weekends.
