About the Role
The Health Core Fellowship Program Coordinator II plans and facilitates the full cycle of the Health Core Fellowship Program, including curriculum design, youth facilitation, and external partnerships for certifications, mentorship, and paid internships. This role involves direct youth engagement and relationship management to guide fellows through a year-long program and into employment or further training.
Responsibilities
- Design and facilitate a phased program structure aligned to health care professional core competencies.
- Coordinate skill and certification trainings (BLS/CPR, First Aid, HIPAA).
- Coordinate site visits to local health agencies and clinics.
- Manage fellows’ enrollment in and completion of City College of San Francisco’s Community Health Worker certificate program.
- Build and maintain a year-round pipeline of paid CHW internship placements with local health agencies.
- Serve as the primary point of contact for community and institutional partners.
- Represent the organization at external events and community meetings.
- Recruit, screen, and retain mentors with clear expectations and milestones.
- Plan and facilitate engaging mentor-mentee events.
- Monitor mentor-mentee engagement and troubleshoot issues.
- Recruit program participants through targeted outreach and a structured interview process.
- Design and monitor a retention and accountability system for fellows.
- Collect program data and maintain excellent recordkeeping.
- Track and report program outcomes, including certificate completion, internship placement, and post-program employment or training.
- Support events hosted or attended by the organization.
- Maintain high professional standards of performance, demeanor, communication, and appearance.
- Perform tasks and responsibilities in a complete and timely manner.
- Support with other related responsibilities as needed.
Requirements
- Ability to engage, inspire, and educate teenage and young adult audiences.
- Ability to facilitate workshops with large groups.
- Knowledge of the Community Health Worker (CHW) model, scope of practice, and core competencies.
- Demonstrated ability to build and sustain institutional partnerships.
- Project management skills to run multiple concurrent workstreams.
- Good communication skills, both verbal and written.
- Strong interpersonal skills, including empathy, humility, and conflict resolution.
- Experienced in Microsoft (Word, Excel, PowerPoint) and/or Google Suite (Doc, Sheets, Slides, Drive).
- Experience in creating designs on Canva is a plus.
- Bachelor’s degree in public health, biology, health sciences, health education, behavioral sciences or related field.
- 3–5 years of combined experience across direct work with transitional age youth and program coordination or partnership/stakeholder management.
- Experience managing partnerships or contracts with external institutions strongly preferred.
- Experience in community organizing or community development is a plus.
- Ability to lift and carry 25 lbs.
- Valid driver’s license and access to reliable transportation.
- Availability to work weekends if appropriate.
