About the Role

Assist in coordinating services for individuals in residential, day habilitation, or community habilitation programs, emphasizing independence, integration, individualization, and productivity. Promote a person-centered approach and ensure services foster growth towards greater independence.

Responsibilities

  • Remain awake and provide active supervision during shifts.
  • Complete documentation, including daily logs, nursing notes, and incident reports.
  • Perform assigned cleaning and household duties.
  • Respond to the needs of individuals supported.
  • Ensure the health and safety of all residents.
  • Assist individuals in managing personal funds, purchases, and disbursements.
  • Assist individuals with nutrition, meal planning, preparation, and grocery shopping.
  • Provide guidance in cleaning, domestic tasks, and upkeep of personal spaces.
  • Ensure the program site is maintained in a safe and sanitary manner.
  • Provide guidance in leisure activities and assist with transportation arrangements.
  • Conduct and document fire drills.
  • Communicate individual and household supply needs to management.
  • Participate in staff meetings and discussions.
  • Participate in crisis intervention.
  • Implement and follow through on ADL, interpersonal skill training, and behavioral support programs.
  • Administer medications if designated and certified.
  • Report any illness, accident, or changes in medical status.
  • Ensure the needs of individuals are met, promoting integration, individualization, independence, and productivity.
  • Communicate program and physical plant matters to management.
  • Accept temporary or permanent transfers as needed.
  • Follow policies and procedures outlined in the Functional Plan.
  • Document service plan documentation per regulations.
  • Monitor individuals during sleeping hours and attend to their needs.
  • Perform daily cleaning duties as required.
  • Act as designated in charge in the absence of the Residence Manager if required.
  • Follow correct procedures for the use of personal funds.

Requirements

  • Must be authorized to work in the United States.
  • Must work a minimum of 8 weekend hours/month.
  • Must work a minimum of 1 agency observed holiday/year.
  • May require approval to work hours outside their primary site.
  • Must be cleared by the transportation department to drive an agency vehicle and maintain driving certification.
  • If hired as non-driver or certification is lost, there will be a reduction in pay.
  • Must successfully complete the OPWDD approved medication course if administering medications.
  • Must maintain medication administration certification.
  • Must use the appropriate documentation procedure and follow instructions for reporting illness or accident.
  • Must adhere to all requirements of the UDSP job description and agency requirements.
  • Must remain aware of and follow policies and procedures.

Salary/Compensations

  • Sign on Bonus- $650, Full time is payable after 3 months of $300 and $350 after 6 months. Part time is half the amount.
  • Sleepers are compensated at the applicable NYS minimum rate for scheduled sleep hours.
  • Base wage is determined by education and experience.
  • Additional Incentive Pay Opportunities: Medication Certification Incentive & Van Certification Incentive.
