About the Role

Assist in coordinating services for individuals in residential, day habilitation, or community habilitation programs, emphasizing independence, integration, individualization, and productivity. Promote a person-centered approach and proactively support individuals' growth towards independence.

Responsibilities

  • Remain awake and provide active supervision during shifts (Awake Overnight).
  • Complete documentation and assigned cleaning duties.
  • Respond to the needs of individuals supported and ensure their health and safety.
  • Remain on-site and available to respond to emergencies during scheduled sleep hours (Sleeper Overnight).
  • Assist individuals in purchasing household items and managing personal funds.
  • Assist individuals with nutrition, meal planning, preparation, and grocery shopping.
  • Provide guidance and instruction in cleaning, domestic tasks, and property upkeep.
  • Ensure the program site is maintained in a safe and sanitary manner.
  • Provide guidance in leisure activities and assist with transportation arrangements.
  • Conduct and document fire drills as required.
  • Communicate individual and household supply needs to the Residence Manager.
  • Participate in staff meetings and discussions regarding individual and household needs.
  • Participate actively in crisis intervention.
  • Implement and follow through on ADL, interpersonal skill training, and behavioral support programs.
  • Document program implementation accurately and on the same day.
  • Administer medications if required, after completing the OPWDD approved medication course.
  • Report any illness, accident, or changes in an individual's medical status.
  • Complete Nursing Notes daily to reflect resident health status monitoring.
  • Write incident reports as defined by Incident Review Procedures.
  • Ensure the needs of individuals supported are met, considering integration, individualization, independence, and productivity.
  • Communicate matters concerning the individual's support program and physical plant to the Residence Manager or Assistant Residence Manager.
  • Accept temporary or permanent transfers within the department as needed.
  • Adhere to policies and procedures outlined in the Functional Plan.
  • Document service plan documentation per regulations and training.
  • Monitor individuals during sleeping hours and attend to their needs.
  • Perform daily cleaning duties as required.
  • Act as designated in charge in the absence of the Residence Manager, if required.
  • Follow correct procedures for the use of personal funds, ensuring proper purchasing and documentation.

Requirements

  • Must work a minimum of 8 weekend hours/month.
  • Must work a minimum of 1 agency observed holiday/year.
  • Requires approval to work hours outside their primary site (Sub Status).
  • Authorized to work in the United States.
  • Must be cleared by the transportation department to drive an agency vehicle and maintain driving certification.
  • If hired as non-driver or certification is lost, there will be a reduction in pay.
  • Must be progressive and proactive in providing services.
  • Must adhere to all requirements of the UDSP job description and agency requirements (Sub Status).

Education Level

  • High School Diploma or equivalency preferred.

Salary/Compensations

  • Sign on Bonus- $650 (Full time: $300 after 3 months, $350 after 6 months; Part time: half the amount).
  • Sleepers are compensated at the applicable NYS minimum rate for scheduled sleep hours.
  • Additional Incentive Pay Opportunities: Medication Certification Incentive & Van Certification Incentive.

Benefits

  • Full time is payable after 3 months of $300 and $350 after 6 months.
  • Part time is half the amount.
