About the Role

The Registered Nurse provides professional nursing care to patients, families, and caregivers within the Medical Group. This role involves promoting, assisting, and advocating for patient and family health through the nursing process, supporting a patient-centered approach and acting as an active care team member. The nurse collaborates with physicians, providers, and multidisciplinary teams, assessing, planning, promoting, and evaluating quality patient care in accordance with state regulations and professional standards. Additionally, the role includes providing patient and family teaching and health promotion, adhering to the mission, vision, and values of Mohawk Valley Health System.

Responsibilities

  • Manages the nursing care process and delivers nursing care services in accordance with New York State nurse practice act, regulatory standards, and MVHS policies.
  • Identifies patient care requirements, establishes rapport with patients, families, and caregivers, and understands the plan of care.
  • Provides emotional, psychological, and spiritual support to patients, families, and caregivers.
  • Promotes patient independence by establishing patient care goals and teaching disease process, health promotion, preventative care, medication management, and self-care skills.
  • Collaborates with care team members and providers to support and adjust the plan of care.
  • Ensures quality care by adhering to standards and measuring health outcomes against patient care goals.
  • Follows the MVHS and Medical Group patient-centered care model and standards of care.
  • Manages nursing care safety and environment of care processes.
  • Protects patients, families, and employees by adhering to infection control policies, protocols, and medication administration regulations.
  • Utilizes standard precautions at all times.
  • Maintains equipment, supplies, and inventory as appropriate.
  • Maintains a safe and clean work environment.
  • Documents patient care services accurately and timely in patient medical records.
  • Maintains continuity of care by communicating the plan of care and following through with the multidisciplinary team.
  • Maintains patient confidentiality and a private environment.
  • Anticipates and maintains needed supplies, equipment, and medications.
  • Maintains professional and technical knowledge by attending educational workshops, mandatory in-services, reviewing publications, and participating in committees.

Requirements

  • Minimum of one (1) year of experience in Medical Office Practice, Outpatient Setting or Specialty.
  • Current New York State licensure as a Registered Nurse.
  • Basic Life Support Certification - Must receive BLS certification within 6 weeks of hire date.
  • Successful candidates might be required to undergo a background verification with an external vendor.
