About the Role

Responsibilities

• Conduct interviews with clients to assess their needs and challenges, such as personal and family adjustments, financial difficulties, employment issues, housing concerns, and physical or mental impairments.
• Gather information on medical, psychological, and social factors affecting the client's situation and evaluate their capabilities.
• Provide counseling and support to help clients develop plans to meet their needs, mobilize internal and external resources, and improve their social functioning.
• Monitor client progress, refer them to community resources, maintain accurate documentation, and ensure effective communication with supervisors and funding sources.
• Assist with housing individuals and providing necessary supplies and support to help them maintain their housing stability.

Requirements

• Associate Degree in Human Services or a related field
• Three years of experience working with vulnerable individuals
• Equivalent combination of education and experience will be considered
• Candidates with lived experience are preferred and encouraged to apply
• Valid N.Y.S. Drivers License
• Registered and insured vehicle
• N.Y.S. Child Abuse Registry clearance
• Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities
