About the Role

This role provides comprehensive clinical case management services, including assessments, individual, group, and family treatment, treatment plan coordination, referent relations, discharge planning, and intervention with client support systems.

Responsibilities

• Coordinate census.
• Coordinate patient transfers.
• Oversee caseloads and scheduling of Adult Program Clinical Case Managers.
• Attend team meetings and audit paperwork as needed.
• Oversee treatment planning.
• Evaluate and delegate rules for the Program as put forth by management.
• Address conflict issues with staff and patients.
• Conduct wing meetings.
• Review and modify the program’s services.
• Engage in public relations with other treatment modalities to assess appropriate referrals for adult clientele.
• Maintain a reduced caseload and responsibilities relative to licensure and title.
• Participate in Performance Improvement Initiatives.
