About the Role

The Health Home Plus (HH+) program provides intensive care management services for adults with Serious Mental Illness (SMI) enrolled in a Health Home. This role involves assessing client needs, developing service plans, connecting clients with community resources, and providing ongoing support to promote self-sufficiency and stability.

Responsibilities

• Aid qualified individuals requiring social service assistance.
• Interview and evaluate clients.
• Formulate service plans and goals to meet clients' mental health and medical needs.
• Utilize community resources and referrals.
• Provide intensive case management services to select populations with Serious Mental Illness (SMI).
• Provide case management services for up to 1 year, or longer based on client need.
• Maintain a caseload of up to 20 clients.
• Conduct assessments and develop care plans.
• Provide service referral and navigation.
• Offer crisis intervention as needed.
• Comply with Federal, State, City, and CAMBA security and privacy policies.
• Conduct required field work, seeing clients in-person a minimum of two times per month.
• Conduct two additional telephonic calls per month for a total of 4 encounters.
• Work with clients to overcome barriers and assist them in self-advocacy.
• Implement strategies to encourage client participation.
• Monitor client progress toward service plan goals.
• Document client progress through service plan outcomes and detailed progress notes.
• Act as a client liaison and advocate with external organizations.
• Escort clients to appointments.
• Follow up with clients after goal completion to ensure stability.
• Market the program to the community to recruit clients.
• Prescreen clients over the telephone for eligibility.
• Schedule appointments for potential enrollment.
• Assist clients in completing applications for benefits and entitlements.
• Process applications on behalf of clients as needed.
• Perform other duties as assigned.

Requirements

• A Master’s degree in a qualifying field and one (1) year of Experience; OR
• A Bachelor’s degree in a qualifying field and two (2) years of Experience; OR
• A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two (2) years of Experience; OR
• A Bachelor’s degree or higher in ANY field with either: three (3) years of Experience, or two (2) years of experience as a Health Home care manager serving the SMI or SED population.
• Experience providing direct services to people with Serious Mental Illness, developmental disabilities, alcoholism or substance abuse, and/or children with SED; OR
• Experience linking individuals with Serious Mental Illness, children with SED, developmental disabilities, and/or alcoholism or substance abuse to community services.
• Ability to work with Microsoft Office (Word, Excel, etc.).
• Ability to utilize web-based reporting platforms.
