About the Role

Responsibilities

  • Coordinate and implement comprehensive case management services for pregnant women.
  • Conduct client assessments, develop service plans, monitor services, and provide follow-up.
  • Ensure regular face-to-face contact with clients and maintain confidentiality.
  • Provide screening services for domestic violence and perinatal depression.
  • Offer health education on various topics and facilitate prenatal care appointments.
  • Collaborate with relevant agencies and offer referrals to community resources.
  • Perform accurate documentation and maintain case files.
  • Attend regular meetings with supervisor for guidance, support, and training.

Requirements

  • Associate's degree in Human Services or related field with 2 years of case management experience, or equivalent combination of education and experience.
  • Experience in facilitating educational groups with vulnerable populations preferred.
  • Experience working with expectant mothers and parents of children under two years of age preferred.
  • Must meet all health standards for health department personnel as a subcontracting agency of the Onondaga County Health Department (OCHD).
  • Valid N.Y.S. Driver's License.
  • Registered and Insured Vehicle.
  • N.Y.S. Child Abuse Registry clearance.
  • Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities.
  • Manual dexterity sufficient to work with children/adults in a challenging and active environment.

Skills

  • Proficiency in utilizing electronic database systems.
  • Strong proficiency in MS Outlook and MS Office.
  • Excellent customer service skills.
  • Effective oral and written communication skills.
