About the Role

Join our expanding behavioral health team as a Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with psychiatric experience. This mission-driven, full-time role focuses on improving the emotional health, behavioral stability, and quality of life for medically complex older adults in skilled nursing facilities. Experience with traumatic brain injuries (TBI) is highly valued due to the prevalence of neuropsychiatric conditions. Responsibilities

  • Provide comprehensive psychiatric consultations, diagnostic evaluations, and medication management.
  • Evaluate adult and geriatric residents experiencing depression, anxiety, dementia, and chronic psychiatric illness.
  • Assess residents with traumatic brain injury (TBI) and neurological conditions (strongly preferred).
  • Develop individualized treatment plans addressing cognitive, behavioral, and medical needs.
  • Initiate, monitor, and adjust psychotropic medication regimens for medically complex patients.
  • Perform ongoing psychiatric follow-up visits to evaluate treatment response and behavioral stability.
  • Collaborate closely with facility physicians, nursing leadership, therapists, and social workers.
  • Educate residents, caregivers, family members, and facility staff regarding diagnoses and interventions.
  • Maintain accurate, timely, and compliant documentation within electronic medical record systems.

Requirements

  • Active and unrestricted New York State clinical license.
  • Board Certified PMHNP-BC or Physician Assistant with significant psychiatry experience.
  • Active DEA registration.
  • Strong background in psychiatric assessment, psychopharmacology, and interdisciplinary healthcare.
