About the Role
This fully remote position offers an experienced and autonomous Nurse Practitioner (NP) or Physician Assistant (PA) the opportunity to provide high-level clinical support to seniors with an exceptional work-life balance. The role involves a 7 days on / 7 days off schedule, combining Provider of the Day responsibilities with after-hours On-Call coverage for a high-risk geriatric population.
Responsibilities
- Conduct telemedicine visits for acute and urgent patient concerns
- Manage same-day virtual evaluations to help prevent unnecessary ER visits and hospitalizations
- Review urgent clinical issues, laboratory and diagnostic results, and coordinate appropriate follow-up care
- Support providers and nursing staff with acute care questions, treatment recommendations, and escalated triage concerns
- Manage medication refill requests and other time-sensitive clinical needs
- Assist with transitions of care and hospital discharge follow-up
- Facilitate timely patient communication and overall care coordination
- Support clinical workflow needs across the practice while collaborating closely with providers, nurses, and operational staff to ensure high-quality patient care
- Conduct after-hours telemedicine visits as clinically appropriate
- Address urgent patient concerns and symptom management remotely
- Provide clinical guidance and treatment recommendations
- Determine appropriate escalation of care, including emergency evaluation when necessary
- Manage time-sensitive medication concerns and refill requests
- Support patients, families, facilities, and staff during urgent situations
- Coordinate urgent follow-up care and communication with the clinical team
- Document all after-hours encounters appropriately within the EHR
Requirements
- Completion of an accredited Nurse Practitioner or Physician Assistant program
- Active and unrestricted NP or PA license
- Must be licensed in Pennsylvania, New York, New Jersey, and Delaware
- Willingness to obtain licensure in additional states as the organization expands (licensure costs reimbursed by the company)
- Experience in geriatrics, family medicine, internal medicine, or emergency medicine preferred
- Prior telemedicine experience preferred
- Strong clinical judgment and ability to work independently
- Excellent communication, organization, and problem-solving skills
- Comfortable managing complex geriatric patients remotely and collaboratively
- Ability to thrive in a fast-paced, team-oriented environment
