About the Role

The Housing Coordinator supports San Francisco Independent Living Program (SF ILSP) participants in finding and maintaining stable housing. This role involves partnering with County Social Workers and other supporters to review housing options, acting as an intermediary between landlords and participants, and primarily working with older youth transitioning out of the foster care system. The coordinator will address housing-related barriers, provide case management for at-risk youth, offer resources and education on housing procurement and retention, and conduct workshops to support independent living.

Responsibilities

  • Conduct workshops on topics including Transitional Housing for Non-Minor Dependents, Housing Choice Voucher, local coordinated entry systems, Move-In Move-Out Best Practices, Good Neighbor/Tenant Habits, Tenant Rights and Fair Housing, Housing Resources, and how to get on Affordable Housing Lists.
  • Perform assessment, planning, and implementation of services to help participants identify and achieve short and long-term goals.
  • Provide direct participant support based on identified needs.
  • Create and deliver housing plans focused on outcomes in wellness, budgeting, long-term housing, education, employment, parenting, relationships, and overall health.
  • Coordinate with referral agencies, identify community resources, track participant progress, and develop creative ways for participants to practice independent living skills.
  • Prepare clients for housing maintenance post-program exit or assist in identifying alternative housing options.
  • Support participants in developing and practicing social-emotional learning skills for conflict resolution and decision-making.
  • Establish new linkages with community-based organizations and housing stability support through case management.
  • Assist participants with moving into supported housing, including securing and transporting furniture, shopping for necessities, and locating community resources.
  • Conduct apartment inspections to ensure cleanliness standards.
  • Display and disseminate relevant community resources and provide eviction prevention support.
  • Schedule client meetings as needed for successful transition to independent living.
  • Coordinate community events and workshops to assist youth with independent living skills.
  • Participate in weekly coordination meetings with ILSP and County staff and other agency partners.
  • Attend weekly program meetings, monthly agency staff meetings, and individual weekly supervision.
  • Maintain participant files and update paperwork monthly.
  • Ensure proper and timely documentation of services, including written case notes.
  • Collaborate with colleagues across the organization and within the department.
  • Perform other duties as assigned.

Requirements

  • Minimum two years of experience working in affordable housing, homeless services, property management, child welfare social services, or with transition-age youth with complex presentations including chronic trauma.
  • Strong dedication to cultural competency that removes barriers.
  • Comfortable working in community settings.
  • Ability to pass a Community Care Licensing background check.
  • Ability to work with a high level of detail, professional polish, and clear, well-thought-out documents and products.
  • Organized and up-to-date database management.
  • Clarity on project goals and personal drive to accomplish them, delivering thorough projects on time.
  • High level of personal accountability for the quality and impact of work.
  • Proficiency with Microsoft Office applications (Word, Excel, Access, PowerPoint, Project, Outlook).
  • Experience with database systems is a plus.
  • Exceptional oral and written communication skills, including the ability to research, assimilate, and analyze information, compose engaging messages, and present data clearly and inspiringly.
  • High level of professionalism in work and interactions, presenting as organized, informed, and friendly.
  • Well-honed ability to independently anticipate and analyze situations, define problems and objectives, recognize viable alternatives, and formulate rapid solutions, understanding inherent risks and implications of tough decisions.
  • Availability for occasional evening and weekend work and some travel as needed.
  • Local candidates preferred.
