About the Role

The New York licensed LCSW collaborates with fellow treatment team members to ensure the provision of quality care tailored to the unique needs and care levels of each patient within an outpatient treatment setting.

Responsibilities

  • Receive, review, and promptly respond to intake referrals upon assignment of the worker's compensation client case.
  • Conduct Bio/Psych/Social assessments, diagnostic impression, and formulate treatment plans under the guidance of a qualified Clinical Supervisor.
  • Meet for individual sessions with patients at a frequency determined by the ongoing assessment of progress in treatment.
  • Complete and assist clients in aftercare and discharge planning.
  • Provide case management services as needed, including referrals to outside resources when necessary.
  • Ensure cohesion of all services documented within the clinical chart and assist with compliance of various standards.
  • Maintain accurate, thoroughly documented client records in the Electronic Health Record (EHR).
  • Attend and participate in treatment team meetings as scheduled.
  • Attend staff development and general staff meetings.
  • Communicate with Medical Director and other prescribers to ensure quality clinical services.
  • Participate in and attend supervision/consultation with the Clinical Director.
  • Collaborate with the multidisciplinary treatment team to ensure effective delivery of clinical services.
  • Comply with local, state and federal confidentiality laws and regulations.
  • Collaborate with professionals both within and outside the agency, as appropriate.
  • Demonstrate sensitivity and responsiveness to cultural differences.
  • Abide by PAX Health code of ethics and model values.

Requirements

  • Licensed Clinical Social Worker licensed in NEW YORK STATE.
  • 5 years' experience as a fully licensed LCSW.
  • Must be able to follow the model of 30-minute sessions with Workers' Compensation clientele.
  • New York State Workers' Compensation Board authorization number required upon offer acceptance.
  • Ability to work as part of a comprehensive community mental health team and represent the agency.
  • Familiarity with community support systems, health care and human service resources, court and police activities.
  • Good organizational skills and administrative abilities.
  • Flexibility in days and hours available for scheduled work, including weekends.
  • Knowledge of and ability to adhere to a professional code of ethics.
  • Knowledge of and ability to comply with the state's mental health code.
  • Appropriate professional registration/certification by state/national authority required.
