About the Role

Catholic Charities of Onondaga County is seeking an experienced Intake Coordinator to join their SSVF services team, serving homeless or at-risk Veterans and their families in Onondaga County. This role is crucial in helping individuals and families overcome challenges related to housing, employment, and personal well-being.

Responsibilities

• Interview clients to determine the nature and degree of problems related to personal and family adjustments, child welfare, finances, employment, food, clothing, housing, and physical/mental impairments.
• Secure information on medical, psychological, and social factors contributing to a client's situation and evaluate these factors along with client capacities.
• Counsel clients individually, in families, or in small groups regarding plans for meeting needs.
• Aid clients in mobilizing inner capacities and environmental resources to improve social functioning.
• Meet with clients as required by funding sources.
• Help clients modify attitudes and behaviors by increasing understanding of self, personal problems, and their role in creating them.
• Monitor client progress.
• Refer clients to community resources and other organizations.
• Network with community service providers.
• Complete all necessary reporting forms for government agencies and funding sources, including UCRs, progress notes, referrals, and statistics.
• Maintain client case files.
• Maintain appropriate communication with supervisors and other funding sources.
• Notify of any safety issues in a timely manner.
• Maintain guidelines established by funding sources and Catholic Charities policies and procedures.
• Perform other duties related to housing individuals and helping them remain housed.

Requirements

• Bachelor’s Degree in a Human Services related field required.
• Three years of experience working in housing and/or with vulnerable individuals.
• A combination of education and relevant experience may be considered.
• Experience in housing, housing law, and other related areas.
• Knowledge of State and Federal Veteran assistance programs.
• Valid N.Y.S. Driver's License.
• Must have a registered and insured vehicle.
• N.Y.S. Child Abuse Registry clearance required.
• Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities.
