About the Role

The Critical Time Intervention Mental Health Professional provides comprehensive care management and coordination of services to individuals with severe mental illness and a history of homelessness during transitional periods. This role enhances continuity of care from hospital to community settings, addressing clinical and rehabilitative needs to support recovery goals.

Responsibilities

• Provide therapeutic interventions for mental health and substance use disorders.
• Educate individuals about mental health, treatment, and recovery.
• Teach coping skills for specific symptoms and stress management.
• Develop a crisis management plan.
• Develop a relapse prevention plan, including identification of early warning signs and rapid intervention strategies.
• Create relationships with key providers in the community.
• Perform other duties as assigned by supervisor.

Requirements

• Qualified Health Professional (including but not limited to LMSW, LCSW, LMHC, etc.) required.
• Minimum of one year of clinical experience in an appropriate program setting.
• Master’s Degree in a related human service field required.
• An equivalent combination of training and experience may be substituted for educational requirements.
• Must have a valid NYS Driver’s License.
