About the Role
The CTI Case Manager provides comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management focused on helping individuals with severe mental illness (SMI) and a history of homelessness during transitional periods, enhancing continuity of care from hospital to community.
Responsibilities
- Carry a primary caseload providing CTI services
- Develop person-centered care plans
- Share knowledge of and provide linkage to resources
- Engage individuals in skill building
- Offer crisis management
- Collaborate with other community providers
- Assist individuals in navigating complex systems
- Work with service recipients’ informal supports
- Perform other duties as assigned by supervisor
Requirements
- One year employment experience in the Human Services field
- An equivalent combination of training and experience may be substituted for educational and employment requirements
- Experience and/or training in care coordination for those with severe mental illness and substance use disorders preferred
- Basic computer skills required
- Must have a valid NYS driver’s license
