About the Role

The CTI Licensed Practical Nurse provides comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. This role supports individuals with severe mental illness (SMI) and a history of homelessness during transitional periods, enhancing continuity of care from hospital to community. The LPN resolves medical and rehabilitative needs within their scope of practice to help individuals achieve recovery goals.

Responsibilities

  • Carry a primary caseload providing CTI services.
  • Assess basic health and medical needs within their scope of practice.
  • Coordinate health care with community medical providers.
  • Provide rehabilitation, education, and support services.
  • Works to create relationships with key providers in the community.
  • Other duties as assigned by supervisor.
  • All client visits are expected to be completed in the community, meeting people where they are at.

Requirements

  • Must be a licensed practical nurse currently registered by the New York State Education Department.
  • Experience and/or training in the treatment of severe mental illness and substance use disorders preferred.
  • Basic computer skills required.
  • Must have a valid NYS driver's license.
