About the Role
The RN Care Services Supervisor manages client care efficiently and cost-effectively to achieve positive patient outcomes. This role operates within the Licensed Home Care Service Agency (LHCSA) and is dedicated to participating in the Home Health Aide Training Program (HHATP), teaching new staff and following them from the classroom to the field to ensure seamless application of learning to patient care. The supervisor develops the overall plan of care for clients and oversees the outcome of that care, including care delivered by other staff.
Responsibilities
- Provide and coordinate quality patient care while maintaining an assigned caseload/productivity.
- Complete accurate and timely documentation supporting care provided and/or client needs.
- Conduct admission assessments to determine a safe plan of care, including the need for home health aide/personal care assistance, visit frequency, and duration.
- Case manage primarily aide-related services according to regulatory guidelines.
- Conduct in-home supervisory visits to oversee care given by home health aides and personal care aides.
- Practice according to best practice standards and regulatory nursing standards.
- Maintain relationships and clear communication with MLTC case managers and other care team members.
- Communicate effectively with aides, providing mentoring and positive feedback through phone conversations and visits.
- Oversee clinical skills of assigned aides and provide training.
- Communicate with aide preceptors regarding skills and potential support needs.
- Document according to standards of nursing practice.
- Contact the primary physician and/or notify the clinical supervisor per regulatory guidelines.
- Mentor new hires with consistent oversight.
- Provide leadership, supervision, and operational guidance to Home Health Aides and Personal Care Aides.
- Review Aide productivity, performance management, employee relations, and disciplinary/counseling.
- Assist in completing annual performance evaluations.
- Investigate and address Aide concerns, complaints, accidents, and other issues involving peers, patients, and/or care delivery.
- Report issues to the Regional Director, Executive Director, and/or Human Resources as appropriate.
- Assist the clinical educator in executing actions for Aide development of knowledge and skills to ensure proficiency and patient care quality.
- Ensure all assigned staff complete annual in-service credits.
- Assist in coordination of the HCR Preceptor program.
- Demonstrate competencies related to information/technology.
- Establish and maintain relationships with community partners.
- Perform other duties as assigned.
Requirements
- Current, valid RN license issued by NYS Department of Education required.
- Minimum of two (2) years’ experience in home care.
