About the Role
The Case Planner is responsible for delivering services to children in care and their families, facilitating permanency goals through counseling and service coordination. This role involves representing the agency in court proceedings and ensuring contract mandates are met for the assigned caseload.
Responsibilities
- Conduct a minimum of one monthly contact with the foster child in the foster home to assess safety, well-being, and progress towards permanency.
- Conduct two face-to-face contacts with biological parents monthly, including one in their home, to discuss ASFA timeframes and assess progress towards the permanency plan.
- Monitor and coordinate visitation plans, ensuring at least one weekly parent-child visit of 2 hours, unless court-directed otherwise.
- Continuously re-assess visitation plans to ensure the least restrictive option based on child development and case progress.
- Schedule and participate in Family Team Conferences.
- Continuously re-assess permanency plans in light of ASFA timeframes and develop concurrent plans.
- Maintain regular contact with biological parents' service providers to ensure they are addressing placement reasons.
- Maintain regular contact and coordinate all child services, including medical, clinical, and educational.
- Maintain accurate and contemporaneous CNNX records with progress notes entered within 48 hours of contact.
- Complete and submit FASPs and Permanency Hearing Reports on time.
- Report, testify, and present the agency's position in Family Court proceedings, including Permanency Hearings and TPR.
- Participate in mandated ACS/Abbott House trainings.
- Perform all necessary activities assigned to further the agency's mission.
Requirements
- Must have a valid Driver’s License and available car for field visits, or be able to use public transportation.
- Must be able to use a computer and learn software programs.
- Must be able to operate a fax, copier, scanner, and other office equipment.
- Must be able to speak, read, and write fluently in the English language.
- Must be able to write in the English language using correct spelling, grammar, and punctuation.
- May frequently lift 0-25 lbs.

- May frequently push/pull 0-25 lbs. of force.
