About the Role

The TFFC Case Planner is responsible for delivering social work services to foster families, natural families, and children in TFFC programming. This role involves providing direct therapeutic services, counseling, and coordinating community and school activities to help families cope with issues leading to child placement.

Responsibilities

  • Provide professional counseling and treatment services to assigned children on a regular basis.
  • Coordinate and monitor all community services for the child.
  • Assist in the placement of the child in school and represent the agency at CSE meetings.
  • Maintain communication with the child's school regarding educational performance.
  • Provide professional counseling and treatment services to the natural family regularly.
  • Coordinate and supervise family visits to assess the family's readiness for the child's return.
  • Model and reinforce appropriate parenting techniques.
  • Link and assist the natural family with community services to facilitate the child's return home.
  • Coordinate discharges and/or transfers and related services for the child and family.
  • Make diligent efforts to work with the natural family towards the child's return, documenting these efforts.
  • Meet with foster parents regularly to discuss the child's progress, monitor behavioral charts, and assist in crisis resolution.
  • Provide foster parents with notice of training and support groups.
  • Attend and participate in foster parent support groups and complete the Parent Skills Training Program.
  • Assist foster parents in coordinating medical, school, and therapy appointments, providing transportation if needed.
  • Develop and maintain a positive working relationship with contracted LDSS workers.
  • Inform the Supervisor of any issues that arise regarding cases.
  • Maintain an up-to-date case record for each child on the caseload, available for review and audit.
  • Complete all DSS mandated paperwork within established time frames.
  • Coordinate and attend Family Team Conferences (FTC)/ Service Plan Reviews (SPR) and court functions with LDSS/ACS liaisons.
  • Follow up all agreements with LDSS/ACS with a confirming letter.
  • Attend all court dates and be prepared to present case record information.
  • Assist in the re-certification of all foster homes.
  • Work closely with the TFFC-Socio-Therapist to coordinate TFFC services.
  • Participate in weekly supervision with the Supervisor, updating on assigned cases.
  • Attend TFFC team meetings and discuss assigned cases.
  • Attend all in-service training as assigned by the Supervisor.
  • Perform any other related duties as required.

Requirements

  • Bachelor's Degree required and/or Master's Degree preferred in Social Work or related field.
  • A minimum of 3 years child welfare experience.
  • Valid driver's license required.
  • Excellent counseling, communication, interpersonal and writing skills.
  • Knowledge of community service providers (agencies).
  • Must be able to use a computer and learn software programs.
  • Must be able to operate a fax, copier, scanner, and other office equipment.
  • Must be able to speak, read and write fluently in the English language.
  • Must be able to write in the English language using correct spelling, grammar, punctuation, etc.
  • Must have a valid Driver’s License and a clean driving record.
  • May frequently lift 0-25 lbs., occasionally lift 25-50 lbs., and rarely lift over 50 lbs.
  • May frequently push/pull 0-25 lbs. of force, occasionally push/pull 26-45 lbs. force, and rarely push/pull over 45 lbs.
  • Will be required to sit, stand and walk throughout the work day.
  • May occasionally stoop.
  • May occasionally kneel.
  • May occasionally crouch.
