About the Role
The Senior Service Coordinator provides engagement and support services to program participants with complex needs requiring intensive support. This role utilizes a person-centered model to help individuals maintain housing, develop goals, further recovery, access community services, build social connections, and explore educational or vocational opportunities. The Senior Service Coordinator also leads group activities, assists the Program Director, and may supervise interns or newer staff.
Responsibilities
- Facilitate a minimum of four groups per month.
- Ensure all services, follow-up, and documentation are completed on time.
- Complete all assessments and service plans on time.
- Enter at least two service plan linked notes for all tenants on assigned caseload within 48 hours of meetings.
- Ensure monthly in-apartment visits for all tenants on assigned caseload.
- Document 40 hours of direct service work each month.
- Provide leadership by facilitating at least one staff training per quarter.
- Provide and document regular individual supervision to at least one intern or staff member.
Requirements
- Understanding, appreciation, and commitment to the philosophy and mission of Community Access, with expertise in recovery-oriented service delivery.
- Minimum of a high school diploma or equivalent (GED) required; bachelor’s degree in a related field preferred (work experience may be substituted for degree).
- Minimum of two years working with individuals with a history of homelessness, substance use, and/or mental health diagnosis.
- Ability to prioritize and meet deadlines.
- Excellent oral and written communication skills.
- Creative and flexible.
- Ability to utilize various computer programs, including Microsoft Word and Excel.
- Ability to show initiative and be responsible for follow-through.
- Ability to maintain confidential information.
- Demonstrated leadership skills and ability to work as part of a team.
- Skilled in conflict mediation/negotiation and assertive problem-solving.
- Must be fingerprinted and cleared through a background clearance process.
