About the Role

Serves as a licensed practitioner responsible for planning, coordinating, delivering, and managing comprehensive health care services in a health care setting. Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable policies, rules, regulations, and standards.

Responsibilities

  • Manages patient care as outlined in practice protocols.
  • Participates in the planning, coordination, and evaluation of practice for patients receiving acute stroke interventions.
  • Obtains thorough histories and performs physical exams to assess patient's physical and psychosocial health status.
  • Provides routine health care maintenance services.
  • Assesses and manages acute and chronic neurologic illnesses in collaboration with a physician.
  • Performs therapeutic measures and diagnostic tests as credentialed.
  • Prescribes medications within the practitioner's specialty as required by law.
  • Establishes medical diagnosis.
  • Records and documents data necessary for healthcare coordination.
  • Initiates referrals/consultations and consults with the attending physician.
  • Promotes safe quality patient care through cooperation and collaboration.
  • Communicates patient's plan of care with appropriate disciplines.
  • Participates in Stroke Teams and evaluates team performance and outcomes.
  • Administers NIH stroke scales.
  • Sees stroke patients anywhere in the organization.
  • Contributes to the success of the stroke center by participating in the call schedule, including nights and weekends.
  • Follows stroke patients ensuring adherence to guidelines for antiplatelet meds, DVT prophylaxis, anticoagulant for atrial fibrillation, lipid measurement, dysphagia screening, NIH Stroke Scale use, stroke education, and rehabilitation referral.
  • Trains professional nurses and other staff in areas related to the Stroke Center's clinical requirements and educational efforts.
  • Makes follow-up calls to patients and arranges appropriate outpatient follow-up.
  • Initiates and participates in studies and research.
  • Participates in hospital committees.
  • Serves as a clinical/educational resource person for clinical personnel.
  • Continually reviews new guidelines and incorporates latest information into clinical practice guidelines and policies.
  • Participates with all staff to achieve zero defects goal for the Hospital.
  • Seeks constructive feedback and acts to achieve goals identified in performance appraisal.
  • Rounds with the clinical team and functions as a resource to clinical professionals and ancillary staff.
  • Helps to ensure compliance with mandated parameters of care.
  • Participates in achieving TJC Certification as a Primary Stroke Center.
  • Champions patient-centered care and the culture of safety.
  • Demonstrates fiscal responsibility in the use of resources.
  • Facilitates system changes to support the use of evidence.
  • Participates in peer review process.
  • Participates in the achievement of Hospital’s Strategic Goals.
  • Evaluates own practice in relation to professional standards and regulatory requirements.
  • Maintains clinical competency by staying abreast of current trends in health care, legislation, regulatory requirements, and nursing practice.
  • Develops, organizes, and participates in health-related research projects and performance improvement studies.
  • Serves as a coach and mentor in evidence-based medicine.
  • Attends national and regional conferences and meetings.
  • Anticipates patients' needs and takes initiative to meet them, keeping patients informed.
  • Provides consistent and timely follow-through for all patient requests.
  • Makes every attempt to defuse potentially volatile situations.
  • Resolves differences in a spirit of cooperation.
  • Maintains composure, efficiency, and positive customer service during peak workload.
  • Adheres to the ICARE standards of Customer Service.

Requirements

  • Minimum of 2 years of experience as a licensed independent practitioner.
  • Current New York State registration as a licensed independent practitioner.
  • Certification by a national certifying body in her/his specialty area.

Salary/Compensations

  • $68.5182 - $91.3576 Hourly
