About the Role

The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are established Manhattan community-based agencies supporting approximately 30,000 New Yorkers. Goddard is a partner within the Manhattan Outreach Consortium (MOC), a borough-wide initiative serving the unsheltered community. The Downtown team serves lower Manhattan, collaborating with community partners and healthcare agencies. The purpose of this position is to coordinate and support the activities of the team’s medical and psychiatric providers to deliver integrated care.

Responsibilities

Manage psychiatric and Street Medicine Team (SMT) schedule, including adding clients and signing in/out with team members.
Ensure Housing Outreach Specialists submit referrals to SMT/psych and facilitate follow-up on recommendations.
Develop linkages with other healthcare providers and resolve insurance issues.
Gather medical information from other appointments or hospital visits.
Assist Housing Outreach Specialists with obtaining medical records from hospitals.
Find providers based on recommendations, geography, and insurance coverage.
Assist with the insurance authorization process.
Consult E-PACES to determine client medical coverage.
Track clients’ Medicaid/Medicare certification for recertification.
Communicate with hospitals and emergency rooms during client acute treatment/hospitalization.
Communicate with Janian providers about emergencies and urgent matters.
Canvass for clients in the field for psych/medical appointments.
Escort clients to medical appointments, labs, and diagnostic imaging centers.
Meet monthly with team leadership and SMT providers to review and update the medically vulnerable list.
Carry a small caseload of the team's more medically vulnerable clients.
Assist SMT with coordination of special projects and public health initiatives.
Perform other duties as required.

Requirements

Bachelor’s degree preferred.
Valid driver's license and a driving record that meets the agency's auto insurance guidelines (no suspensions within the last 3 years).
Flexibility in work hours; available to work evening, overnight, and weekend shifts when required (participation in 4 overnight counts a year).
Proficient in Microsoft Office: Word, Excel, Outlook.
Able to use or learn computer databases such as MOCapp, CARES, E-PACES.
Frequent walking and standing.
Occasional lifting of up to 30 lbs.

Skills

Ability to multi-task/work in a fast-paced environment.
Proficiency in Spanish and English preferred.
Understanding of housing first and harm reduction models.
Experience with homelessness preferred.
Strong organizational and communication skills.
Ability to work as part of a team.
