About the Role
In this rewarding role, you will play an integral part in helping older Australians live independently and safely in their own homes. Working closely with clients, families, and service providers, you will coordinate personalised supports that enhance well being, promote choice, and deliver exceptional care outcomes.
Responsibilities
- Manage a diverse portfolio of Home Care clients, supporting individuals with needs ranging from entry-level assistance through to complex care requirements.
- Develop, implement, and regularly review personalised care plans that reflect each client’s goals, preferences, and evolving needs.
- Coordinate a range of services and supports, ensuring clients receive high-quality, client-centred care.
- Build strong and trusted relationships with clients, families, healthcare professionals, and community partners.
- Monitor client funding and budgets, prepare care budgets, and ensure services are delivered within funding parameters.
- Action changes in care requirements, maintain accurate records, and ensure compliance with relevant legislation and quality standards.
- Contribute to local business growth through community engagement, relationship building, and promotion of RSL LifeCare services.
- Collaborate with internal teams and frontline staff to ensure the consistent delivery of outstanding customer service and care.
Requirements
- Demonstrated experience working within the Support at Home, Home Care Package, or Aged Care funding environment.
- Current CPR Certificate
- Qualifications in individual support or related work experience are beneficial
- Strong understanding of client-centred, community-based care delivery and the associated aged care quality standards.
- Proven experience managing and monitoring budgets and financial expenditure.
- Ability to develop and maintain productive relationships with clients, staff, external providers, and key stakeholders.
- Excellent communication, organisational, and problem-solving skills.
- Current Australian Driver’s Licence and access to a fully insured vehicle.
- A valid Police Check or a valid National Disability Insurance Scheme (NDIS) check, or potentially both, to undertake the inherent requirements of the role. Clearances must be verified prior to commencing employment with RSL LifeCare.
