About the Role

The Clinical Resource Manager is responsible for the timely facilitation and coordination of patient care for a specific patient population or area of assignment. The role promotes high quality, efficient, and cost-effective care by assessing patient needs and facilitating care that meets established patient needs while optimizing reimbursement.

Responsibilities

  • Facilitate and coordinate patient care for a specific patient population or area of assignment.
  • Work effectively with the interdisciplinary team to plan and implement an individualized safe and appropriate discharge plan.
  • Assess patient needs utilizing established standards, Care Maps, and other approved clinical guidelines.
  • Communicate and coordinate with third-party representatives.
  • Facilitate care that meets established patient care needs.
  • Optimize reimbursement.

Requirements

  • Bachelor's degree required.
  • Current registration in New York State as an RN, Diploma and Licensure required.
  • At least 5 years of clinical experience as a Registered Nurse.
  • Experience with utilization review and discharge planning.
  • PRI and Screen certified.
  • Knowledge of Microsoft Word and Excel required.

Skills

  • Interqual knowledge/experience preferred.
