About the Role
The Service Coordinator provides engagement and comprehensive support services to program participants in Community Access’ supported housing programs. This role supports individuals using a person-centered model to help them maintain housing, develop goals, further recovery, access community services, connect with support networks, and explore educational or vocational opportunities. Service Coordinators are responsible for individual support and community building group activities.
Responsibilities
- Facilitate a minimum of four groups per month.
- Ensure all services, follow-up, and documentation are completed on time.
- Complete all assessments and service plans on time.
- Enter at least two service plan linked notes for all tenants on assigned caseload (8 in licensed housing) within 48 hours of meetings.
- Ensure monthly in-apartment visits for all tenants on assigned caseload (weekly in Community Residences).
- Document 40 hours of direct service work each month.

- Be creative and flexible.
- Ability to utilize various computer programs, including Microsoft Word and Excel.
- Show initiative and be responsible for follow-through.

- Ability to work independently and as part of a team.
- Skilled in conflict mediation/negotiation.
- Assertive approach to problem solving.
- Must be fingerprinted and cleared through a background clearance process.
