About the Role
Utilizing clinical experience, this role coordinates patient services under the direction of the team manager/designee. Key responsibilities include prioritizing and coordinating daily patient schedules, requesting additional services, and ensuring timely assignment of referrals.
Responsibilities
- Validates supplemental orders and processes per policy.
- Remains available to assist in other duties deemed appropriate to the scope of practice of an LPN.
- Manages multiple phone calls in a timely and professional manner providing appropriate follow up.
- Provides field staff assistance and resources for day to day issues.
- Checks email in a timely manner providing follow up.
- Collects, organizes and reviews various programs for information and reporting and initiates follow up.
- Updates the EMR per agency standards.
- Performs functions related to transfers or discharges as required by regulation.
- Builds and maintains strong, communicative relationships with the team.
- Provides exemplary customer service.
- Performs general office duties including answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies.
Requirements
- Graduate of an accredited school of licensed practical nursing.
- A valid NYS license as a Licensed Practical Nurse.
