About the Role
The Division of Cardiology at Albany Medical College is seeking a full-time Nurse Practitioner or Physician Assistant for their outpatient cardiology practice. This role offers the opportunity to provide high-quality cardiovascular care in a supportive, team-oriented environment, working closely with cardiologists and multidisciplinary care teams. The position will focus on outpatient follow-up visits and pre-operative cardiovascular evaluations, ensuring continuity of care for patients with various cardiac conditions.
Responsibilities
- Conduct outpatient follow-up visits for patients with cardiovascular conditions such as coronary artery disease, heart failure, arrhythmias, hypertension, and valvular heart disease.
- Perform pre-operative cardiovascular evaluations and risk assessments for patients undergoing surgical procedures.
- Obtain detailed medical histories and perform focused cardiovascular examinations.
- Order and interpret diagnostic studies including laboratory tests, ECGs, and cardiac imaging as appropriate.
- Develop and implement treatment plans in collaboration with supervising cardiologists.
- Provide medication management and patient education on cardiovascular disease prevention and management.
- Coordinate care with primary care providers, surgeons, and other specialists.
- Document patient encounters and treatment plans accurately in the electronic health record.
- Participate in quality improvement initiatives and practice efficiency efforts.
Requirements
- Current license and active registration as a Nurse Practitioner or Physician Assistant in New York State.
- Master’s degree from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program.
- Interest in or experience with cardiovascular medicine preferred.
- Familiarity with electronic medical records; Epic experience preferred.
- Strong communication and collaboration skills in a multidisciplinary clinical environment.
- Commitment to patient-centered care and maintaining HIPAA confidentiality standards.
