About the Role

The Division of Cardiology at Albany Medical College is seeking a full-time Nurse Practitioner or Physician Assistant for their outpatient cardiology practice. This role offers the opportunity to provide high-quality cardiovascular care in a supportive, team-oriented environment, working closely with cardiologists and multidisciplinary care teams. The position will focus on outpatient follow-up visits and pre-operative cardiovascular evaluations, ensuring continuity of care for patients with various cardiac conditions.

Responsibilities

Conduct outpatient follow-up visits for patients with cardiovascular conditions such as coronary artery disease, heart failure, arrhythmias, hypertension, and valvular heart disease.
Perform pre-operative cardiovascular evaluations and risk assessments for patients undergoing surgical procedures.
Obtain detailed medical histories and perform focused cardiovascular examinations.
Order and interpret diagnostic studies including laboratory tests, ECGs, and cardiac imaging as appropriate.
Develop and implement treatment plans in collaboration with supervising cardiologists.
Provide medication management and patient education on cardiovascular disease prevention and management.
Coordinate care with primary care providers, surgeons, and other specialists.
Document patient encounters and treatment plans accurately in the electronic health record.
Participate in quality improvement initiatives and practice efficiency efforts.

Requirements

Current license and active registration as a Nurse Practitioner or Physician Assistant in New York State.
Master’s degree from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program.
Interest in or experience with cardiovascular medicine preferred.
Familiarity with electronic medical records; Epic experience preferred.
Strong communication and collaboration skills in a multidisciplinary clinical environment.
Commitment to patient-centered care and maintaining HIPAA confidentiality standards.
