About the Role

As a Licensed Practical Nurse, you will provide direct nursing care to acutely and chronically ill patients in their homes, record clinical notes, and coordinate patient care activities to ensure the highest quality of service within your assigned geographic area. You will work under the direct supervision of a Registered Nurse or designated supervisor.

Responsibilities

• Provide high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members, following all applicable infection control procedures.
• Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record using the agency’s electronic visit verification program (as applicable).
• Actively participate in the agency’s care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, other clinicians, agency management, and case coordinators/community case managers (as applicable).
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Requirements

• Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to work flexible hours.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Candidates are required to pass a drug test before beginning employment.

Skills

• Current CPR certification or ability to complete within 90 days of hire.

• $24.10 to $36.15 per hour
