About the Role
By The Bay Health is seeking a Hospice Nurse Case Manager to cover San Francisco. This role involves providing skilled nursing assessment, planning, and care to maximize patient and family comfort and health, working as part of an interdisciplinary team.
Responsibilities
- Provide care in the home and work as a member of the interdisciplinary team comprised of nurses, social workers, chaplains, hospice aides and physicians.
- Coordinate all patient/family services and prioritization of needs, including implementation of the individualized plan of care.
Requirements
- One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
- Experience in acute care/ home health and/or hospice setting preferred but not required.
- Current license as a registered professional nurse in the State of California.
- Public Health Nursing certification highly desirable.
- HPNA certification preferred.
- Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
- By The Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Salary/Compensations
- $65.70 - $75.70 hourly
