About the Role
Lighthouse Guild, a leading not-for-profit healthcare organization, seeks a Part-Time Licensed Physical Therapist for its GuildCare program in Manhattan. This program provides services to adults who are blind or visually impaired and have chronic health care needs.
Responsibilities
- Evaluate, plan, develop, and implement physical therapy services as ordered.
- Ensure therapy personnel and registrants follow established policies and procedures.
- Review physical therapy policies and recommend changes.
- Maintain a working relationship with registrant’s community physician.
- Review requests for physical therapy and physician's orders, interview and evaluate registrants to determine treatment and schedule therapy.
- Participate in the development and implementation of registrant care plans, assessments, and discharge planning.
- Comply with established safety precautions.
- Complete necessary forms, reports, and submit to the Program Director.
- Maintain treatment records and progress notes per GuildCare policy.
- Recommend equipment and supply needs to the Program Director.
- Ensure the therapy area is clean and safe, with necessary equipment and supplies.
- Ensure all therapist notes are informative and descriptive of care provided and registrant response.
- Complete service logs accurately and in a timely manner.
- Perform physical demands including standing, walking, lifting/transferring registrants, using lifting equipment, bending, stooping, squatting, twisting, reaching, pushing/pulling wheelchairs, and lifting/carrying equipment/supplies.
- Perform general assistance tasks including escorting registrants, assisting with transportation, serving meals, cleaning up from meals, and filling in for absent coworkers.
- Maintain the confidentiality of registrant protected health information.
- Follow the LGI Workforce security policy and access information systems and ePHI authorized by Clinical Authorization only.
- Observe and implement the Registrants' Bill of Rights.
Requirements
- Master's degree with New York State license and certification is required.
- Experience in a hospital/clinic/long term care setting and with the needs of the visually impaired population is preferred.
