About the Role
Dialysis Clinic, Inc. is seeking a Pediatric Dialysis Social Worker to provide individualized care for patients facing chronic kidney disease. This role involves assessing patient needs, counseling on psycho-social, financial, and transportation issues related to ESRD, and working within a pediatric dialysis program.
Responsibilities
- Counsel patients and families regarding ESRD therapies and the financial aspects of ESRD modalities.
- Assist patients in completing financial forms.
- Ensure patient transportation by supplying appropriate information and making contacts.
- Participate in operational programs, including staff and in-service meetings.
- Gather and document information to assess and modify the care plan in accordance with regulatory requirements.
- Compile and develop educational materials for patients and staff.

Salary/Compensations
- Starting at $30 per hour
