About the Role
Family Case Planners help strengthen families, stabilize and maintain family functioning by assisting caretakers to meet children's needs, prevent abuse/neglect, and enhance parenting, coping, communication, and insight skills.
Responsibilities
- Perform initial assessments of family needs pertaining to placement risk factors, problems, needs, and capacity to use preventive services.
- Develop preliminary service plans in coordination with supervisors.
- Conduct community outreach and advocacy.
- Provide case planning, family counseling, and other casework modalities under supervision.
- Plan, coordinate, and deliver subordinate services to families with the assistance of a Family Aide.
- Monitor linkages with community service providers and visit schools.
- Secure the safety of all children served.
- Facilitate client use of psychiatric and psychological services and follow clinician recommendations.
- Conduct at least three face-to-face contacts monthly (two office visits and one home visit).
- Perform outreach for resistant clients.
- Conduct two yearly contacts with school officials for school neglect cases.
- Perform a second home visit if a child under 6 months is in the home.
- Monitor and supervise case management of all preventive services.
- Complete all FASPS on time, including weekly progress notes.
- Perform administrative tasks related to caseload, including statistics, recordings, FASPS, progress notes, and contact logs.
- Participate actively in weekly individual and group supervisory conferences.
- Conduct Parenting Skills Training groups and Adolescent and Children's Groups.
- Perform other tasks as needed.

- Ability to use public transportation for travel to program locations throughout New York City.
