About the Role

Support various programs, engage with referred individuals, and provide holistic, person-centered support to empower clients and promote positive change. Sensitivity to each individual’s unique circumstances, cultural background, mental health, and substance use challenges is central to this role.

Responsibilities

• Enroll individuals and complete intake and documentation.
• Develop individualized, person-centered action plans to address clients’ specific needs.
• Connect individuals to essential community resources and conduct follow-ups as needed.
• Process and maintain Social Care Network billing, as well as other billing and reimbursement responsibilities.
• Meet regularly with participants to monitor/document progress and assist in addressing barriers and goal progression.
• Build and maintain relationships with community organizations to facilitate resource access.
• Perform other duties as assigned.

Requirements

• Familiarity with community resources and case management practices for high-risk populations.
• Bi-lingual Spanish proficiency preferred.
• Strong interpersonal, engagement, and communication skills.
• Proficiency with computer skills.
