About the Role
Support various programs, engage with referred individuals, and provide holistic, person-centered support to empower clients and promote positive change. Sensitivity to each individual’s unique circumstances, cultural background, mental health, and substance use challenges is central to this role.
Responsibilities
- Enroll individuals and complete intake and documentation.
- Develop individualized, person-centered action plans to address clients’ specific needs.
- Connect individuals to essential community resources and conduct follow-ups as needed.
- Process and maintain Social Care Network billing, as well as other billing and reimbursement responsibilities.
- Meet regularly with participants to monitor/document progress and assist in addressing barriers and goal progression.
- Build and maintain relationships with community organizations to facilitate resource access.
- Perform other duties as assigned.
Requirements
- Familiarity with community resources and case management practices for high-risk populations.
- Bi-lingual Spanish proficiency preferred.
- Strong interpersonal, engagement, and communication skills.
- Proficiency with computer skills.
