About the Role

The Peer Specialist provides engagement and support in the community and/or office setting as a member of the PACT team. This role includes assisting with risk assessments, brief interventions, care management, and follow-up support. The position is designed for individuals with lived experience of mental health recovery and requires providing excellent participant care and compassion.

Responsibilities

• Provides support and care to those engaged in PACT.
• Identifies healthy community opportunities for individuals to socialize and interact; promote these opportunities as directed by the care coordinator or other care provider.
• Connects and supports individuals throughout the 36-week program.
• Assists in the delivery of evidenced based interventions.
• Assists in the development of and adherence to community safety plans.
• Participates in team meetings and trainings as required.
• Understands PACT Team policy and procedure, Agency and regulatory policies and procedures.
• Participates in the writing and timely completion of any required documentation in accordance with state/federal regulations and agency and program policies and procedures.
• May be assigned to train and supervise other staff as deemed appropriate.

Requirements

• Ability to obtain OMH Peer Certification within 90 days of employment.
• Complete OHM Partnering for Safety & CPR training within 90 days of employment.
• Extensive knowledge with PC applications and electronic medical records strongly preferred.
• Valid NYS driver’s license and reliable transportation required for local and regional travel.
