About the Role

The Outpatient Social Worker provides customized, evidence-based clinical interventions to treat the root causes of psychosocial and environmental symptoms. This role serves as a liaison between medical professionals and patients to enhance collaboration and improve medical outcomes.

Responsibilities

• Provides crisis intervention with an immediate safety action plan for patients with suicidal, homicidal, hallucinating, substance abuse, and interpersonal violence/abuse issues.
• Conducts Social Work assessments to identify the root cause of psychosocial and environmental symptoms.
• Creates care plans and connects patients with services to address their needs.
• Manages referrals for psychosocial needs and determines appropriate interventions and strategies.
• Facilitates behavioral change using clinical interventions such as Motivational Interviewing, Collaborative Care, Cognitive Behavioral Therapy, Behavioral Activation, and Problem-Solving Therapy.
• Develops customized interventions for specialty populations.
• Completes diagnostic screening tools including PHQ, Edinburgh, CAGE, and GAD 7.
• Diagnoses patients using the DSM V.
• Documents all services in accordance with clinical guidelines and policies.
• Must have the ability to travel for CompassionNet.

Requirements

• Master’s Degree in Social Work
• NYS LMSW or NYS LCSW
• One (1) to three (3) years of social work experience in a health care or home care setting
• End-stage renal disease experience (dialysis)
• Compassionate, warm, and patient-focused demeanor
• Exceptional documentation and planning skills
• Excellent communication and interpersonal skills
• Minimum one (1) to three (3) years pediatric and/or palliative care experience for CompassionNet.
