About the Role

The Clinical Therapist will provide clinical services, primarily individual and group therapy and case management care to patients in a hospital-based outpatient mental health clinic setting. Experience treating concurring substance abuse and mental illness is required for this position. Documentation must be completed in a timely and quality manner.

Responsibilities

• Provide intake assessments, individual and group therapy, collateral and family involvement, crisis intervention, and patient/family psychoeducation.
• Complete documentation including person-centered treatment plans, progress notes, assessments, screening tools, and results in a timely manner.
• Follow designated timeframes for documentation as identified in department policy.
• Complete high-risk documentation including CSSRS and safety plans in accordance with department policy.
• Provide appropriate clinical services that are person-centered and trauma-informed.
• Provide and engage in collateral work with outside agencies, programs, and services.
• Efficiently incorporate case management duties to enhance patient care.
• Work with the interdisciplinary team, including staff meetings and high-risk meeting involvement.
• Communicate and collaborate with other professional staff.
• Engage in the supervision process/meetings and apply feedback to clinical work.
• Continue professional development by attending a minimum of 4 trainings per year.
• Meet productivity expectations of 9 daily visits.
• Demonstrate effective time management, including adhering to work shift schedules and managing patient appointments, scheduling, and documentation.
• When clinically appropriate, complete toxicology screenings and necessary interventions based on UDS results.
• Adhere to monitoring and documentation expectations around the treatment of dually diagnosed individuals.
• Adhere to department policy regarding outreach calls/letters, documentation of missed visits and gaps in treatment, collateral contact, and program consents.
• Provide psychoeducation to patients and families around the use of NARCAN, overdose education and prevention, and medication-assisted treatment.

Requirements

• Master's degree required.
• NYS License/Registration as a Social Worker required (LMSW/LCSW).
• Prior experience in a hospital or community health agency is preferred.
• Experience with assigned patient population and utilization of PM Schedule.
• Experience treating concurring substance abuse and mental illness required.
• Ability to cover dual locations.
