About the Role
This role involves providing virtual health services, assessing and managing patient conditions, leveraging digital diagnostics, and collaborating with an interdisciplinary team. The position requires maintaining accurate electronic health records and participating in quality initiatives.
Responsibilities
- Assess, diagnose, and determine/alter treatment and management plans via virtual health modalities.
- Order, perform, and interpret diagnostic studies, and prescribe pharmacologic and non-pharmacologic interventions.
- Manage conditions based on clinical indication, evidence-based care, cost-effectiveness, and assessment of risks/benefits.
- Leverage digital diagnostics and remote physiologic monitoring.
- Provide health promotion, disease prevention, and disease management counseling and education.
- Manage patients as part of an interdisciplinary team and within scope of practice.
- Seek consultation or referrals from physicians or other healthcare team members as appropriate.
- Escalate the need for more emergent or specialized care when necessary.
- Maintain accurate, complete, concise, and timely documentation in the electronic medical record.
- Substantiate and submit professional services consistent with compliant coding and billing practices.
- Facilitate consistent, coordinated care and clear communication among all members of the healthcare team and/or health or community agencies.
- Participate in quality, safety, and peer review initiatives/performance activities.
- Participate in organizational and/or departmental meetings and committees, peer review, and workgroups.
- Participate in education and/or onboarding of new team members, students, and other health care professionals.
- Seek experiences to maintain and develop clinical and professional skills.
- Maintain standards of productivity, access, face-to-face time, and quality metrics.
- Provide care appropriate to the age of the patients served, demonstrating knowledge of growth and development principles.
Requirements
- If Advanced Practice Registered Nurse: Active RN, and applicable APRN/NP/APRN-FPA/APNP license(s) in state(s) of practice.
- If Physician Assistant: Active PA license(s) in state(s) of practice.
- If Advanced Practice Registered Nurse: Active national board certification as Nurse Practitioner in area of clinical practice and populations served: Family, Adult and Adult-Gero.
- If Physician Assistant: Active PA national board certification from National Commission on Certification of Physician Assistants.
- All APRN and Physician Assistant applicants must have: Active DEA registration as applicable for state of practice.
- If Illinois practice: active Illinois Controlled Substance License.
- Demonstrated high level of clinical proficiency.
- Demonstrated excellent decision-making skills.
- Demonstrated ability to work independently and as an effective member of a health care team.
- Demonstrated ability to adapt to evolving technology.
- Proficiency with the electronic medical record.
- Excellent communication skills.
