About the Role
The Skill Builder / Vocational Specialist will assist participants in achieving a recovery lifestyle and meet the vocational rehabilitation needs of the participants. This role is based out of the Circare program and works Per-Diem.
Responsibilities
- Provide all appropriate CORE/HCBS services: Psychosocial Rehabilitation (PSR), Prevocational Services, Ongoing Supported Employment, Education Support Services, Empowerment Services - Peer Supports (if applicable)
- Facilitate individual and groups/trainings covering assessment of vocational, social, and educational needs
- Facilitate individual and groups/trainings covering referrals for needed external services
- Facilitate individual and groups/trainings covering job readiness preparation and training
- Provide rehabilitation counseling, skill building, and psychoeducational interventions to support attainment of person-centered recovery goals and valued life roles
- Promote independence and full community participation
- Individual Recovery Planning
- Provides ongoing job development and readiness training
- Discharge Planning
- Teach and encourage independent living skills to promote mental wellbeing and recovery
- Collaborate with community organizations and agencies to enhance Vocational and Living Skills programming
- Collaborate with Care Managers and other care team providers as necessary
- Performs Quality Assurance activities including providing services according to agency policies and procedures and governmental regulations/guidelines
- Performs Quality Assurance activities including evaluating programming and individual needs to identify strengths or barriers
- Performs Quality Assurance activities including assisting in problem resolution and implementing appropriate program modifications
- Performs Quality Assurance activities including participating in committees as assigned
- Performs Quality Assurance activities including maintaining appropriate program records and statistical accountability
- Represents Helio Health and the program to the community
- Other tasks as assigned by supervisor, including tasks falling under the umbrella of services provided in Community Oriented Recovery and Empowerment, Home and Community Based Services and ACCES-VR programs
Requirements
- One year of employment experience in the Human Services field preferred
- Vocational rehabilitation counseling experience required
- Equivalent alternative combination of education and experience will be considered
- Must have a valid driver’s license and own transportation
- Employees who drive personal vehicles for work must maintain minimum auto insurance coverage limits of at least $100,000/person; $300,000/accident and provide HR with proof of current minimum insurance coverage
- Employees are required to provide an updated Auto Liability Insurance Declaration Page to HR by expiration date
- Minimum physical effort is required, with occasional lifting up to 30 pounds
- Ability to bend, stand, sit, stoop, reach, push and pull on a daily and/or regular basis
- Ability to sit/stand for extended periods of time consistent with administrative and presenting duties
- Finger manipulation and hand coordination for constant computer use
- Ability to pass a test for illicit substances
