About the Role

Concierge Nursing Referral Services connects elite physical therapists with discerning, high-net-worth clients seeking exclusive, in-home care. Focus on delivering high-touch, unhurried, personalized therapy without the friction of insurance constraints.

Responsibilities

• Design and implement highly customized treatment plans tailored to your client’s lifestyle and functional goals.
• Provide hands-on, high-quality interventions to maximize patient recovery and independence.
• Coordinate directly with physicians, family members, and multidisciplinary care teams to deliver a cohesive client experience.
• Guide and train clients and their families to maintain long-term safety, wellness, and functional mobility.
• Maintain clear, prompt documentation in accordance with referral standards.

Requirements

• Active Physical Therapist (PT) license in the State of California (Required).
• 2+ years of PT experience strongly preferred; prior home health or private duty experience is a major plus.
• Proven ability to work autonomously with exceptional clinical judgment.
• High reliability, flawless communication, and a polished, professional demeanor suited for VIP clientele.
• Tech-savvy with basic computer skills for quick, efficient digital charting.
• Proof of eligibility to work in the U.S.
