About the Role

The Case Manager will work with the interdisciplinary team to support clients in the community, participating in team meetings and providing regular updates to managers. The role aims to assist in developing service care plans, accessing appropriate services, and providing case management through client intake, needs assessment, and consultation. This involves direct service provision using a strength-based, trauma-informed approach, developing and monitoring service plans, conducting follow-ups, advocating for clients, and connecting them with community resources.

Responsibilities

  • Advocate on behalf of the client
  • Connect clients with community resources
  • Establish effective working relationships with staff of other agencies

Requirements

  • An undergraduate degree or diploma in social services or a related field
  • A minimum of three years’ experience in direct service delivery to adults with serious psychiatric disabilities
  • Strong knowledge of crisis intervention/prevention, addictions, recovery-oriented practice, culturally competent and trauma-informed care, and severe/persistent mental illness
  • A solution-focused approach
  • Strong crisis management skills
  • Experience working with diverse communities from an anti-racism, anti-oppression, and non-homophobic lens
  • Ability to work both independently and within a multidisciplinary team
  • Effective analytical, assessment, and decision-making skills
  • A valid Ontario Driver’s License
  • Access to a vehicle
  • Appropriate insurance coverage
  • Adherence to COVID-19 policy and proof of vaccination
