About the Role

The OASIS Nurse is responsible for analyzing data integrity and consistency of OASIS documentation and assessment processes. This role educates and collaborates with clinical staff to clarify documentation and data integrity issues, ensuring appropriate coding and sequencing.

Responsibilities

  • Performs other duties as assigned.
  • Reviews OASIS clinical assessment documentation for appropriateness, accuracy, completeness, and compliance.
  • Consults with clinical staff and provides educational guidance on code selection, OASIS data item responses, documentation, and Plan of Care development.
  • Ensures documentation is consistent with the patient's condition and the reason for services provided.
  • Captures clinical severity for the level of service provided to ensure quality, cost-effective care and compliance.
  • Collaborates with Agency leadership to address trends affecting the Agency's outcome and process measures.
  • Provides education and acts as a resource to clinicians, supervisors, office staff, and billing staff regarding OASIS C requirements, CMS guidelines, home care ICD-10 coding, PPS guidelines, and billing issues.
  • Assists the Finance Department in creating a Home Health Resource Group (HHRG) and Health Insurance Prospective Payment (HIPPS).
  • Assists Agency leadership with achieving corporate goals, analyzing data, improving patient care outcomes, and patient satisfaction.
  • Participates in Agency quality assessment/performance improvement initiatives.

Requirements

  • Bachelor's Degree in Nursing.
  • Current and valid NYS Registered Professional Nurse license and registration.
  • 1-2 years of Home Health care experience.
  • Working knowledge of CMS, OASIS, and ICD-10 Coding requirements.
  • Knowledge of Federal and NYS Department of Health regulations.
  • Knowledge of The Joint Commission requirements.
  • Ability to promote and strengthen continuous quality improvement concepts.
  • Working knowledge of computer systems and electronic medical records.
  • Good communication and interpersonal skills.
  • Critical thinking skills.
  • Meticulous attention to detail.
