About the Role

Extended Home Care of NY is seeking an experienced and reliable Occupational Therapist to conduct home visits for IDD patients. The occupational therapist will provide therapy services according to a written physician's plan of care, including assessment, evaluation, and participation in the total plan of care.

Responsibilities

  • Assess occupational therapy needs by applying objective diagnostic and prognostic tests.
  • Prepare initial assessment of the patient’s condition and perform periodic re-assessments.
  • Develop a plan of care in accordance with the authorized practitioner’s prescription for therapy services, including service type, estimated duration, and visit frequency.
  • Ensure the patient’s plan of care includes clearly defined, measurable short and long-term goals with time frames.
  • Provide occupational therapy treatment according to established goals, in collaboration with the patient’s physician.
  • Document the patient’s response to treatment and modify the treatment plan and objectives as needed after notifying the patient’s physician.
  • Instruct healthcare personnel, the patient, and family in techniques for safe management and maximum restoration of mobility and function.
  • Complete case conferences, 60-day summaries, and discharge summaries when indicated.
  • Prepare all documentation on appropriate forms.
  • Participate in the development and periodic revision of the physician’s Plan of treatment with the Home Care Coordinator.
  • Participate in case conferences, staff meetings, and in-service programs as needed.
  • Collaborate with the Interdisciplinary Team to develop appropriate interventions for clinical and functional goals.
  • Participate in the agency’s Quality Assurance Performance Improvement/PIP as assigned.
  • Submit visit notes at least weekly, or within 7 days after completion of each home visit.
  • Provide written Home exercise programs to clients and the Home Care Coordinator, and instruct patient/family/caregiver as per physician’s instructions.
  • Demonstrate sound judgment by taking appropriate actions regarding suspected violations of corporate compliance regulations.
  • Report all suspected violations to supervisor, Compliance Officer, or Compliance Hotline.
  • Perform other duties as assigned.

Requirements

  • Graduate of an Occupational Therapy school approved by The American Occupational Therapy Association and The Council on Medical Education and American OT Association of the Council on Medical Education of Hospitals for the American Medical Associate.
  • Possess a License and current registration to practice as a Registered Occupational Therapist in New York State.
  • Two (2) years of institutional and/or home care experience required.
  • Satisfy state health status requirements.

Salary/Compensations

  • $100,000.00 - $120,000.00 per year (Full-time)
  • $80 - $100 per visit (Fee For Service)
